Thank you for visiting Integrated Medical Services’ (“IMS” or “We” or “Us”) internet website at www.imsaz.org (the “Site”). Integrated Medical Services is committed to protecting the privacy and security of the Users of the Site. For information about individual Integrated Medical Services practices, please contact that particular practice.  IMS is based, and the Site is hosted, in the United States of America.  If User is from the European Union or other regions of the world with laws governing data collection and use that may differ from U.S. law, and User is visiting, accessing or otherwise using the Site, please note that any personally identifiable information that User provides to IMS will be transferred to the United States.  Any such personally identifiable information provided will be processed and stored in the United States by IMS or a service provider acting on its behalf.  By providing personally identifiable information to IMS, User hereby specifically and expressly consents to such transfer and processing and the uses and disclosures set forth herein.

IMS does not intentionally share personally identifiable information with third parties without your consent, provided, however, (i) We may share some or all of your personal identifiable information with a third party to complete your request for payment of goods or services via the Site; (ii) We may share some or all of your personal identifiable information with a third party to complete your request for information; (iii) We may disclose personally identifiable information at the request of law enforcement or governmental agencies or in response to subpoenas, court orders or other legal process, to establish, protect or exercise IMS’ legal or other rights or to defend against a legal claim or as otherwise required or allowed by law; (iv) We may disclose personally identifiable information in order to protect the rights, property or safety of a User or any other person; (v) We may disclose personally identifiable information to investigate or prevent a violation by User of any contractual or other relationship with IMS or the perpetration of any illegal or harmful activity; (vi) We IMS may also disclose aggregate, anonymous data based on information collected from Users to investors and potential partners; (vii) We may disclose or transfer personally identifiable information collected from Users in connection with or in contemplation of a sale of its assets or business or a merger, consolidation or other reorganization of its business. If you have any hesitation about providing such information to us and/or having such information displayed on the Site or otherwise used in any manner permitted in this Privacy Statement or in the Terms of Use, You should not use the Site.

 

If you upload personal identifiable information or otherwise provide content, You grant to IMS the right to access and use the personal identifiable information and the content that You upload. We do not monitor individual use of health information resources, such as KramesStaywell® Knowledgebase or Integrated Medical Services health information pages.

 

Even after you have requested information from Us and have provided Us with personally identifiable information such as that discussed above, You may later choose not to continue to directly receive further email communications from IMS by sending an email message to admin@imsaz.org stating that you no longer want to receive email communications directly from IMS (“Opt Out”). Once IMS has received your request to Opt Out, IMS will use commercially reasonable efforts, in a commercially reasonable time period, to carry out your request to Opt Out; provided, however, We will not have any obligation to retrieve, and cannot prevent the further use by others (including, but not limited to, any of the IMS Partners), any of your personal identifiable information that may have been shared with others up to that time.

 

If You elect to Opt Out, and provided You have properly delivered your Opt Out request to IMS in accordance with this Privacy Statement, You will no longer receive emails directly from IMS after a commercially reasonable time from the date you have sent your Opt Out notice to IMS. However, IMS does not have any obligations under, or liability arising out of, this Privacy Statement with regard to: (a) your use of any other internet websites (including, but not limited to, any online payment solution services), blogs, bulletin boards, interactive areas, and similar forums which are not under IMS’ control and ownership; and (b) any other communications sent to You by any party other than directly from IMS.
